    The numbers to call and the deadlines that start running the moment something goes badly wrong on site. Print it, laminate it, put it in the truck.

    Life-threatening injury Call 911
    Then Make the area safe, but do not disturb the scene beyond that
    Then Tell the employer or GC the same day, and get it written down

    OSHA reporting deadlines

    Work-related fatality Report within 8 hours
    In-patient hospitalization Report within 24 hours
    Amputation Report within 24 hours
    Loss of an eye Report within 24 hours

    Being told to do something unsafe

    First Raise it with the employer, in writing
    If nothing changes File with OSHA at osha.gov or 1-800-321-OSHA, and ask them to keep your name off it
    Retaliation Illegal under Section 11(c) of the OSH Act
    Flat refusal Legally protected only in narrow cases. Read the guide before you walk off
    The fatality rule applies if death occurs within 30 days of the incident. The hospitalization rule applies if it happens within 24 hours of the incident, and being kept only for observation or diagnostic tests does not count.
